As Ayman Abu Shamalah mourns the loss of his wife and children, he watches newborn Mecca in an incubator at a hospital in Rafah in the southern Gaza Strip. She was saved by an emergency caesarian section after her mother - and older siblings - was killed in an Israeli bombing on 21 October.
